Care and Support Planning
Source Care are here for you every step of the way, after you have made contact with us or we have received a referral from you social worker, we will ask to come and meet you.
Our aim is to make sure you are 100% involved in planning your care. We believe in the “nothing about me, without me” approach. This is known as being Person Centered.
A care manager will arrange to have a visit with you at a convenient time. They will discuss with you the areas you need support in, how we can help, and how often you need support. We will also do what’s called a Mental Capacity Assessment if you need this.
We then sit together and make a plan. This is called a Support Plan. You will always have access to your plan, and things may change as you do!
We can write all the things you want to achieve in here, and you work toward this with your chosen staff member, we call them Support Workers. They are specially trained people who understand your needs and help you figure out your goals, and how you’ll get there.
